If you’re working in a Student Support Team (SST), Problem Solving Team, or Professional Learning Community (PLC), understanding the function behind a student’s behavior is key to developing effective interventions. Our Simple Functional Behavior Assessment (FBA) Form is designed to guide your team through that process—without requiring a full formal FBA.

This easy-to-use tool helps your team identify and describe a specific behavior of concern, analyze when and where it typically occurs, and explore what the student may be trying to gain or avoid. By organizing environmental and functional insights, teams can more confidently match strategies to student needs and develop a team-based Behavior Intervention Plan (BIP) that actually works.
